Step 2: Create your WordPress quiz form

Step 3: Set up quiz scoring
Open the Settings tab at the top of the form builder, then go to Actions & Notifications. This is where you tell Formidable Forms what kind of quiz this is.
You'll see two quiz actions: Scored Quiz and Quiz Outcome.

Scored quiz

From the same screen you can weight certain questions more heavily, deduct points for wrong answers, and randomize the question and answer order with one click, which is handy for online exams where you don't want answer keys circulating. Click Update to save.
Outcome quiz
An outcome quiz is what powers product finders and personality quizzes like "Which Hogwarts house are you?" Add the Quiz Outcome action, then define your outcomes and which answers point to each one.

Each outcome gets its own result content, so you can add images, video, text, links, or a button pointing to the product that matches the result.

Step 4: Add the quiz to a WordPress page or post
All that's left is publishing. Open the page or post where you want the quiz, click the + icon in the block editor, and add the Formidable Forms block. Pick your quiz from the dropdown and it appears right in the editor.

Hit Publish or Update, and your quiz is live.
