How to create an interactive quiz in WordPress
To start creating an interactive quiz in WordPress, you'll need Formidable Forms.

Next, install and activate the plugin. Then it's four easy steps to follow:
1. Create your quiz form and add questions
Inside your WordPress dashboard, go to Formidable → Forms → Add New.
Next, you have three options to create your quiz:
- Create a blank form from scratch
- Use a pre-built quiz template
- Use the AI form builder
Whichever option you choose, Formidable Forms will open its drag and drop form builder.
Here, you can add your form fields to start customizing your quiz. On the left-hand side, you can edit the field's settings. This is also where you can add images for the answers.
While building your form, you can add conditional logic to your quiz.
So, if you want to show/hide question types based on previous answers, you can easily do so from here. And you can set the different quiz question types from here, whether it's multiple-choice, fill-in-the-blank, etc.
The Randomize options button also randomizes questions for users, so the experience is different every time.
Once your quiz is set up, it’s time to set up the quiz feature.
2. Change quiz outcomes
The quiz outcomes decide what a user will see upon completing the quiz and giving their "correct answers." And it's what makes Formidable's quiz builder so powerful.
To set it up, go to Settings → Actions & Notifications → Quiz Outcome.
Inside the quiz settings, give your quiz action an Action Name. No one will see this besides you, and it’s only there for easy reference.
Next, you can customize the following:
- Customize the action name
- Choose when the action is triggered
- Upload an image to show the results
- Add results you want to display in the Visual/Text editor
- Set additional outcomes
- Add the conditions for the outcome to be met
Some of this isn't necessary to change, but it's worth knowing what it can do. But the main thing we want to look at is #8, adding outcome conditions.
3. Define the outcome logic
Now, we want to set up the outcome paths for your visitors.
Within the Quiz Outcome settings, you can adjust how your quiz will handle questions and answers. Click Add Outcome Conditions to customize the settings.
Here, you'll choose the question along with the answer.
Then, based on this answer, you can set the outcome based on their chosen answers. For example, the above set of answers would result in a 1950s-era answer.
Feel free to customize this exactly how you want. The Formidable Visual/Text editor lets you get creative, so spend time customizing it to get it exactly right!
Once you’ve customized these settings, click Update to save.
4. Display your quiz
To display the quiz, go to your WordPress website and the page or post where you want your form to show.
Next, add a new block and search Formidable Forms. Click the Formidable WordPress block, then choose your quiz from the dropdown menu.
The other option is to use a piece of shortcode.
Go to Formidable → Forms and click <> underneath Actions to use this.
Then, select Insert manually and copy the WordPress shortcode.
Last, go to your page or post and paste the shortcode into a Shortcode block.
