Then, install and activate it, and it's five simple steps:

Step 1: Activate the Survey add-on

After you've installed Formidable Forms, go to Formidable → Add-Ons in your WordPress dashboard. Then, click Install to get the Surveys and Polls add-on.

Step 2: Create your ranking survey

Next, create your survey like you create a form.

First, go to Formidable → Forms and click Add New in the top right of the page.

Next, choose Create a blank form to start from scratch or choose one of our survey form templates to get started faster.
