    Can I get the member to save a questionnaire and come back to it multiple times before submitting?

      Yes, logged-in members can save drafts and return to the questionnaire at a later date, or even multiple times before they finally submit.

    I am trying to do a questionnaire with a log in feature that will allow them to save and continue later without losing data entered. Also don’t want there to be a link to return but the ability for them to log in and finish using their username and password. Will your plug in do this?

    Hi,
    Can it be used for designing clinical questionnaires for an online pharmacy? For instance, the customer picks the medicine for purchase, then by clicking an action button (Start Consultation) they get forwarded to a page containing a form where they are presented with a set of question, the answers which help the pharmacist to approve the supply request. Once they satisfactorily complete the form, the item will get added to the cart.

    UK Meds website has a good example of this.

      Hi Babak,

      Yes, this is absolutely possible. All you would need is a WordPress site and Formidable Forms (the least expensive paid plan would be enough).

      You would set up a site with a front page with some text and a button that would take them to the form. On the form itself, you could automatically send the info the the pharmacist email address and the customer email address if desired.

      There are a lot of ways to improve the setup, based on your exact needs. There’s no “online pharmacy” template, but this shouldn’t be too difficult to achieve. Once you become a paid customer, you’ll have access to our support team. They will be able to give you all the guidance you need to finish the set up.

    Hi there,

    Would users be able to complete the same questionaire multiple times and see a list of their results when logged in?

    Could completed/submitted questionaires be editable by the user?

    Are they able to delete entries?

    Is a PDF of the results able to be created?

    I never really thought about the differences between quiz, questionnaire, and survey... but I suppose we've built all 3 in Formidable! Another I'd add to the list is a voting system. We've built some real-time event voting sites for people to "grade" a speaker/session and then a scoreboard to tally the results in real-time. Fun stuff!

      You can insert HTML into the field labels and we also have HTML fields, so this should be possible.

    I want to ask 15 questions, each with 5 answers. Each of the 75 answers has a different score when selected to give a total at the end. Is this possible with this plug-in?
    Also, would I be able to grant permission for a user to view the average results of a group of results, such as those of 10 people.
    Would this plug-in allow users to complete the questions monthly and see a history of results?

      Yes, you should be able to do this with Formidable Forms. Depending on the details of your setup and use case, you may need an additional permissions plugin like MemberPress or members to manage who can see what, but generating the average should be possible.

    That's pretty interesting. Is it possible, to use this plugin as kund of a service configurator? Approaching a matching product, by asking questions dynamically depending on the answers? And finally propose a solution.

      Yes this is possible...We have one customer who used formidable forms to collect a wide range of information from a customer, then offer a specific recommendation based on the information. In this case it was specialty vitamins. As long as you can map out what results to show based on the info provided, it should be possible.
